Family Nurse Practitioner
Full Time
The Family Nurse Practitioner (NP) is responsible for direct patient care at rural and remote health centers, via telehealth, in the patient’s home and other locations as determined by the practitioner/patient. The NP supports the primary care team’s delivery of health promotion programs to individuals and groups in a variety of settings in the rural and/or remote area. The NP will deliver programs and services using a holistic model of care and practice in a culturally respectful and safe manner.
Youth Peer Support Worker
Full Time
The Foundry Youth Peer Support (YPS) Worker is a vital member of the Foundry centre team, providing peer-based emotional support, mentorship, and service navigation to youth and young adults. Drawing on their own lived experience with mental health and/or substance use challenges, the YPS Worker helps youth enhance their wellness, access resources, and engage with Foundry and community services. They foster hope and recovery through one-on-one and group support, outreach, and advocacy, while promoting a safe, inclusive, and youth-friendly environment.
